As far as I can tell, it's:

75% coir, 20% perlite, 5% charcoal ... styrofoam peanuts or lava rock in bottom of pot for drainage.

After years of using this for phals (have not found anything better for me yet!) it is mostly a visual estimate - sorry - perhaps I'll do an actual one of these days and surprise myself? 😊

Most important facet is quality of coir - day and night difference in quality ad various brands/products - you want one well cleared of salts and with some fiber present to create air space.

Thank you BOTH for the tip. I think I also found the original post where Mike explained in detail with pictures. Very helpful....I have a phal that needs some rescuing. I tried clay pebbles but I think I was watering it too much. I have hopes this mix will make my orchid happy.

coir/charcoal/perlite mix

I repotted my phals in the mix suggested here. Though they all are showing new growth since, i'm wondering about the mix-either I packed it in too much or my coir is denser than most or I'm a worrywart but it seems like the mix is pretty dense.
Question for everyone here who uses this mix formula-is your mix light and airy? Do you pack it in around the roots or leave it loose?

The coir itself should have a certain amount of stingy fibers - this will help keep it airy as well as the perlite. Some coir is much finer than I would prefer - which would lead me to increase perlite if I had no other options - or add some fibrous material to it. Yes - be careful not to pack too tight. Once you figure out the correct watering frequency it should be fine....better to err on the side of under watering if anything.
