please help, dying phal

Hello,
so the orchid I repotted couple of weeks ago keeps dying.
it seems that the roots keep on rotting and the leaves are limping.
At this point I don't even think that it has any good roots left.
Any suggestions? Is it possible to save it at this point or is it a lost cause.
I am really upset because I do not know the name for it and I really really like this one.

Can you post a pic of what you have left including the rootless bottom?
A number of ideas have been suggested on the forum for dealing with this kind of situation - lots of factors can influence success rate. Eliminate ALL rot from root area - don't leave shortened stubs of roots with pieces of rotten material still on them that can spread.
Ideas of growing new roots I remember reading
1) sphag and bag technique - some swear by it some are more challenged - others can give more details if you want to try - includes putting plant in bag with sphag around the root area.
2) place plant on a tray/plate with the root area covered with a layer of sphag and spray daily to keep area damp.
3) I'd put in a small pot of coir mix and keep dampish - but not soaking.
Include superthrive or other root stimulant in water.
4) If plant has a number of large leaves you might want to consider (and you are not going to like this!) removing the largest one(s) so that there is not so much surface area to dessicate the remaining plant.
5) Mist leaves in the morning hours to help reduce dessication.
6) Cross fingers and toes!
