Leaves turning yellow then brown

Here's an orchid I won 3.5 years ago and it hasn't bloomed yet. I do the same things with all my orchids with worm tea and companion and essentials. I usually soak them once a week in the worm tea bath and add the companion and essentials every other week. All my other orchids seem to love this. Not quite sure what is going on with this one. It gets half day sun, lots of circulation and I usually water everyday. As you can see it is in a basket hanging. Any clues what could be wrong?

Nancy I see that the leaves have fungal spots on them, but if you are saying that there is plenty of airflow to the plant then the spoting shouldn't be a problem. The yellowing looks gradual, but the one leaf in particular.... It's brown and crunchy on the tip.

I have to ask, how often and how much are you fertilizing? This looks almost like fert burn.

Are there any critters? (ie mites?)

Is it the oldest growth? If it is, it could be the way this particular plant decides to toss a leaf.

I was wondering if it could be age. The pseudobulbs are nice and plump but this is the oldest leaves. It is in the basket with just some charcoal and not much else. That is the way it has been for the last 3.5 years, the way it came from the grower. Maybe I need to add sphag?
Do you think I could just cut off the yellow and brown part?
