I dunno about the moss on the top...I have moss on the top of some of my orchids, but they're in bloom, so I won't be repotting mine just yet. Edit: Nevermind, I'm being distracted by a game of scrabble at the donut stop... from the look of that plant it needs repotting. I was talking to the woman who runs the local orchid place, and often orchid dealers will send or reduce in price plants needing repotting so they don't have to do it.

Your milt looks very healthy. It looks like a spike or could be a new leaf. If it is a spike leave it alone until it has finished blooming and starts putting out new growth then repot. I use Better-Gro special orchid mix I get from Lowes and its in a clay orchid pot. When repotting go up 1 pot size. Milts are lushes and love their water. If you start seeing accordian pleats in the leaves it needs more water/ humidity. I do see one flower and they last quite awhile and they fragance is wonderful. I find milts very easy to grow. I have 3 and they sit in my east kitchen window and are very happy. In the summer they go outside in a shaded area and this summer just went nuts growing with all the cool,rainy days we had in June.
