Have you thought about mounting it?

I'll echo Daethen's thoughts. If the plant isn't doing well in a pot no matter the media, I'd try mounting it!

You can certainly save that one, leaves still look firm as well as new root growth which is always a positive sign. Not quite sure why the roots continue to struggle however, the pot size looks to be adequate and as you say your other plants are doing well...
Perhaps allow the media to dry out a little more between watering, if you want to keep it potted you can try a half/half mix of chunky bark and sphagnum, bark on bottom, sphagnum on top (or just mix it all together). This way the media will have the aeration needed and the moisture to encourage new root growth, you'll just have to adjust your watering habits a bit...
I also agree with Jonada and Emma that mounting is almost always a sure fire way for a speedy recovery!!!

I'm in the same boat - I have a phal. that I moved it from 1 room to another - even tho it faces the same direction. I "think" it was a tad too cool for that plant It started going downhill in 2 weeks!! I'm now nursing it and will be trying the mount technic - just have 1 question in wait before venturing into this. (hopefully Emmaye or someone will come back & give me the answer I need on my thread)

I have some phals that came in sphag and had potted them in a bark mix and they protested. So back in sphag they went. Some I did mount and they turned around others didn't. I found a trick with using sphag for the finiky phals.loosen the moss and remove or change the original sphag but pot it up loose so it dries faster at the root ball even add packing peanuts if root ball is massive. But the secret or trick that works best for me is to put them in looser sphag moss in a clay pot it dries more evenly and helps prmote root growth not rot. I've been having success this way with especially a few species that spent their lives in sphag and protest any other medias or mounts I've tried. So far though I have 2 doing wonderfully mounted on a pad of sphag. But keep in mind you have to water more often but hard to over water so if you're a heavy handed water mounts are the way to go for them but loose sphag in clay pots if you wait until sphag is getting on the crinchy side works. Remember they like moisture but not super wet or soggy.
Good luck

if you have a propensitity for overwatering and root rotting plants, then you need to pot in a very large bark mix, with lots of air pockets between the bark chunks....and water only at a two week schedule untill you master it....phals are incredibly tempermental this time of year....and it needs to be in a small pot as possible....gl
