I have the odd plant that does this. Usually I believe it's due to a lack of water when the leaf was developing and emerging. Maybe you've kept it too dry or maybe there roots are bad? Have you checked the roots?

I have had this happen a couple of times with plants that have a great root system and the only thing I can't put it down to is that I haven't watered that particular plant enough.

It happens to Paphs from time to time. Some seem more prone to it than others, and I've heard speculation that triploid, tetraploid or aneuploid plants may be more prone to it. If anything I've seen it most often in plants that are growing vigorously and appear to be especially robust. If it is caused by "something" it is most likely triggered at a very early point in the development of the leaf or even in the meristem and then the pattern is set, and not an indication of any long term problem.

That looks kind of like what happens to oncidiums when they don't get enough water/humidity.

That is interesting, I have some Paph seedlings with this problem. Generally I thought it would be just watering but if it means I have a high ploidy on these seedlings it is very exciting........if they ever flower of course.
