I think my orchid is dying

Ive had this orchid 2 years, it was a gift. The flowers all drop off when we moved house but it was fine other wise. I repotted it in August and it started growing more leaves and i finally got it to flower last november. I went away for a week last month, March, and when i came back it was fine. Then most of the blooms dropped off as well as most of the leaves and the top roots went shrivelled and brown. While I was away we had a severe cold spell after a very mild spell of weather.

The orchid is usually in the conservatory although ive since moved it inside as i thought the sudden change in temperature may have effected it. I usually water it once or twice a week with three ice cubed although i have watered it more then this since it has looked sick.
Can anyone help

A picture would be helpful, if you can post one.

Even without seeing a picture, I would suggest you stop using ice cubes to water this plant. You mention a "severe cold spell" as a possible cause of trouble, so why keep using ice to water? Orchids are mostly tropical plants and appreciate room temperature water.

How are the remaining leaves? Without knowing the type of orchid, it is hard to comment on watering, but I assume it is a phalaenopsis, often sold with instructions to water with ice cubes. If you are watering twice a week in the winter, it is possible you are not letting the roots dry out between waterings. They should turn silvery before you water again. The first thing I would do is take the plant out of the pot and check the root condition.
