Exclamation HELP! Have I killed my orchid?

Hi,
Got my first orchid for my birthday just under 3 months ago. Recently, all the the flowers fell off at first, then leaves have dropped off too. I no have no mor elevated. Iím so concerned that Iíve killed it. I followed the limited instructions it came with, only watered when the soil was dry. I even have the orchid drip feed stuck in the pit. I didnít have it in direct sunlight, it was at the opposite side of my room to the window. I have now moved it to the window. Iíll include some pictures of my orchid when I first got it and now. I think this may be the end for my orchid, I donít know much about it or itís breed as no information came with it whatsoever.
If anyone could help thatíd be great

Yes, a Phalaenopsis. One of the most common Supermarket, Hardware, Flower shop pot plants these days. The flower spike/s in pic are dead, normal. If the leaves from where these spikes have come from are gone, so is the plant. Cold is their main killer, overwatering in a mix that stays too wet is another big killer, also some plants are in containers with no drainage, thus the plant roots drown. If you are given another plant or purchase one, post here & some basic cultural advice can be offered. Just seeing what I am, the best thing to do is pop it in the rubbish & start again.

Photos of the stem don't help much. Can you take pictures of the leaves and roots? Also, what is it potted in? Bark, or some sort of moss (usually sphagnum)?
