Hi and welcome to the forum. You could be overwatering the plant causing root rot; but that may not be the case if the potting media is very well draining. You should examine the roots and if they are brown and mushy then you will have to cut back all the roots that are rotting. If you have anything left in the way of healthy roots there is a chance that the plant might be saved. If the roots are white or green then you have some other trouble. Please let us know the result of the root inpection.
