Why throw the entire plant away? I know that blooming out of the crown is an aborration but do we know for certain that the plant will immediately die afterwards? Have people documented this or is it somethign that everyone has heard of but no one has ever actually seen? Phals, being Vandaceous can start new growths from the base of the plant. So I'm not convinced that the only or best option is to throw it away.

Having said that, what you've got is a wacky Harlequin-type Phal. Here are some links to photo albums featuring Harlequin Phals:

Harlequin Phals 1
Harlequin Phals 2 (only those at the very bottom of the page - this person has mistaken splash petal Phals and Harlequins for being the same which they're not)
Harlequin Phals 3

These pages should give you a long list of names to start with and you can search form there.

Cheers
