definitely repot. toss all the old medium and disinfect the pot if you're going to reuse it.
and don't worry, i end up repotting phals in bloom all the time. doesn't seem to bother them; just try not to break the spike.
(it's time like those when you wish you had a third hand.

oh and i agree about cutting off the spike if the plant's really sickly.
